First and Last Name, TDLR License Number, License Type, Name of School, and T-Shirt Size.  

We cannot guarantee a t-shirt in your size, but we will do our very best!  We would love for you to join us regardless! We will register you and send you an invoice online that must be taken care of asap.  We will accept a PO or credit card payment.  Thank you!
